Our APSW team provides intensive case management and supportive counseling to adults with developmental disabilities who live independently in the community. Clients are referred to the program through central intake at Community Living Toronto. The majority of client appointments are conducted through home visits and arrangements to meet in the community. 

Our team also manages and responds to client crises which vary from evictions to hospitalization, attempts to self harm and involvement with the court system. In addition to counseling and case management, we offer voluntary trusteeship for those unable to manage their financial affairs. A social activity group and art program complement the services.
